Medicare and Advance Health Care Planning

The Medicare and Advance Health Care Planning tools from Southern Maine Agency on Aging are services and resources that can help patients and the hospital community plan ahead for their healthcare needs.

Medicare Information

The most critical element for patients in receiving good healthcare services and maintaining compliance with medication regimes is choosing the best plan for their financial and medical circumstances.  This can be a confusing and challenging process as discussed in The Kaiser Family Foundation Report, How are Seniors Choosing and Changing Health Insurance Plans?. SMAA is available to offer your patients and the hospital community unbiased Medicare information throughout the year for new enrollees and annually during open enrollment.

Unbiased Welcome to Medicare Seminars are offered in a group setting for 60-90 minutes. Group sessions can be followed up with 1:1 counseling session days to assist patients and hospital community in signing up for Medicare.  All sessions are facilitated by trained SHIP Counselors (State Health Insurance Program). While counselors will not recommend or sell insurance programs, they will answer questions and teach participants how to consider their options.

Group programs may be combined with other important healthcare offerings such as vaccination clinics including influenza vaccines. 

What to Expect

  • SMAA can support recruitment of patients via invitation based on patient/member data

  • SMAA handles all registration for events and shares the data with the healthcare provider or insurer

  • SMAA can capture important information during the registration process such as registering for additional services like vaccines when registering for a session

Advance Health Care Planning

Group sessions are available for insured members, patients, and the hospital community along with one on one counseling sessions to develop an Advance Directive Form.

This session is helpful for adults of all ages and will introduce topics such as:

  • What is an Advance Directive and why they are important?

  • How to create an Advance Directive

  • Discussing issues that may surface when developing an Advance Directive

  • How to talk to family and friends about advance care planning

  • What to do with an Advance Directive once you have one

Group programs may be combined with other important healthcare offerings such as vaccination clinics including influenza vaccines.
